Active Ageing

Active Ageing offers quality care and support within the comfort of your home, in social groups or out in the community. We encourage individuals to maintain their independence and make their own choice from a wide range of support services. Individuals and their families are invited to be involved in designing a Support Plan that focuses on working together to gain positive outcomes that enhance and enrich the person’s wellbeing as outlined by the Governments Aged Care Reform, “Living Longer Living Better. programs include social groups, Level 1 and 2 home care packages, breaks for carers and support to access the community to name a few.

In line with the Aged Care Reforms “Living longer, Living Better” our experienced and trained staff work closely with individuals and their families and friends to identify their own unique needs and goals to promote their wellbeing. We offer information and support in selecting the programs and activities that will achieve these results whilst respecting the individuals right to personal choice.

Whether in the home, in social groups or out in the community, we promote services that aim to meet individual needs, choices and standard models of care for:

All of our Aged Care programs have eligibility guidelines and criteria for assessment and referral prior to commencement.
