The Community Care Field Worker is employed to provide in home service as identified in the Care Recipients Care Plan. An effective worker empowers Care Recipients through service provision that offers choice and participation, that is flexible, innovative and most importantly encourages and actively supports people to live enriched and purposeful lives as valued members of the community.
Regular duties may include:
- Direct Care
- Support Care Recipients with activities such as meal preparation, personal hygiene, supervision of medication and other independent living skills
- Support Care Recipients in Social Support Programs
- Provide Support Services
- Provide appropriate assistance to Care Recipients such as transport, support to attend appointments, living skills, maintenance of family connections and other goals set out in their individual Care Plan
Employees are rostered and required to attend mandatory training sessions to enhance professional knowledge and develop the application of best practice techniques, strategies and Policy and Procedures.
