Community Aged Care Packages (CACP)

Community Aged Care Packages provide low level of care for elderly people in their own home to assist them to maintain their independence.  Services provided include personal care, housework, laundry, shopping, transport to local appointments and meal preparation. 

An assessment by the Aged Care Assessment Team (ACAT) is essential to determine eligibility for a package.  Following a referral from ACAT the CACP Coordinator will conduct an individual assessment in order to design a care plan that is tailored to your individual care needs. Field staff have a minimum qualification of Certificate lll in Aged Care. 

A fee for service will be charged in accordance with guidelines set out by the Commonwealth Department of Health & Ageing for this service provision.  However, no one will be denied a service they need, based on an inability to pay fees.

CACP are available in the Bay & Basin and the wider Shoalhaven area.  At present we have 20 funded packages.
